Primary adrenal insufficiency, also known as Addison's disease, is a rare endocrine

disorder that happens when the adrenal gland isn't able to produce enough of the hormones

that the body needs, particularly aldosterone and cortisol.

The reason it's called "primary" is that the underlying problem is localized to

the adrenal gland itself, rather than a problem of a hormone that acts on the adrenal gland

or elsewhere in the body.

Primary adrenal insufficiency can develop acutely or chronically, and a really famous

example of someone having this condition is John F. Kennedy, who was diagnosed at age

30.

Now, there are two adrenal glands, one above each kidney, and each one has an inner layer

called the medulla and an outer layer called the cortex which is subdivided into three

more layers, the zona glomerulosa, zona fasciculata, and the zona reticularis.

The outermost layer is the zona glomerulosa, and it's full of cells that make the hormone

aldosterone.

Aldosterone is part of a hormone family or axis which work together and are called the

renin-angiotensin-aldosterone system.

Together these hormones decrease potassium levels, increase sodium levels, and increase

blood volume and blood pressure.

Aldosterone is secreted in response to elevated levels of renin, and it's role is to bind

to receptors on two types of cells along the distal convoluted tubule of the nephron.

First it stimulates the sodium/potassium ion pumps of the principal cells to work even

harder.

These pumps drive potassium from the blood into the cells and from there it flows down

its concentration gradient into the tubule to be excreted as urine.

At the same time, the pumps drive sodium in the opposite direction from the cell into

the blood, which allows more sodium to flow from the tubule into the cell down its concentration

gradient.

Since water often flows with sodium through a process of osmosis, water also moves into

the blood, which increases blood volume and therefore blood pressure.

The other function of aldosterone is to stimulate the proton ATPase pumps in alpha-intercalated

cells which causes more protons to get excreted into the urine.

Meanwhile, ion exchangers on the basal surface of the cell move the negatively charged bicarbonate

into the extracellular space, causing an increase in pH.

The middle layer is the zona fasciculata, and the cells there make the hormone cortisol

as well as other glucocorticoids.

Cortisol is also needed in times of emotional and physical stress like arguing with a friend

or fleeing from a pack of raccoons.

In those situations, the hypothalamus—which is an almond-size structure which sits at

the base of the brain, releases corticotropin-releasing hormone is released from, and received by

the pituitary gland, the pea-sized structure sitting just underneath the hypothalamus.

In response, the pituitary gland sends out adrenocorticotropic hormone, or ACTH, which

travels through the blood to the zona fasciculata of the adrenal glands and signals cells there

to release cortisol.

Cortisol is a lipid-soluble molecule, meaning it can mingle with fats, which allows it to

easily pass through the plasma membrane of cells and bind to the receptors inside.

In fact, almost every body cell has cortisol receptors, so it affects an huge variety of

functions in the body!

One of cortisol's main jobs though is to increase blood glucose levels by promoting

gluconeogenesis in the liver, gluconeogenesis is the formation of glucose from noncarbohydrate

sources, like amino acids or free fatty acids.

Cortisol also gets the muscles to break down proteins into amino acids and gets adipose

tissues to break down fats into free fatty acids, both of which provide the liver with

more raw materials to work with.

So basically, cortisol keeps blood glucose levels high, and this is in contrast top the

hormone insulin, which causes glucose to be taken up by various body tissues, and so essentially

cortisol acts to counteract this effect this in an effort to make sure that the body can

respond appropriately to those raccoons, or other stressors.

Finally, the innermost layer is the zona reticularis, and cells there make a group of sex hormones

called androgens, including one called dehydroepiandrosterone, which is the precursor of testosterone.

So the adrenal glands are involved in testosterone production in both men and women, but the

amount that the adrenals contribute is pretty small, relative to the testes in men, which

accounts for the very different levels of androgens in men versus women.

In men, high levels of androgens are responsible for the development of male reproductive tissues

and secondary sex characteristics like facial hair and a large larynx or Adam's apple.

In women, low levels of testosterone are responsible for a growth spurt in development, underarm

and pubic hair during puberty, and an increased sex drive in adulthood.

The exact mechanism for adrenal androgen production is not well understood, but like cortisol,

it seems to be stimulated by adrenocorticotropic hormone released from the pituitary gland.

So, that all brings us back to primary adrenal insufficiency, which is where the adrenal

cortex gets progressively damaged over time.

In developed countries the most common cause is autoimmune destruction, when the body's

own immune cells mistakenly attack the healthy adrenal cortical tissues, though the precise

reason why this happens isn't clear.

In developing countries the most common cause is tuberculosis; in this case the infection

spreads from the lungs to the adrenal glands, causing inflammation and destruction in the

adrenal cortex.

Another important cause is metastatic carcinoma, which is where cancer spreads to the adrenal

cortex from somewhere else in the body.

Regardless of the cause, it turns out that the adrenal cortex has a high functional reserve,

meaning that a small amount of functional tissue can still do a pretty decent job of

churning out enough of the hormones to meet the body's needs.

As a result of this though, once there are symptoms, it's usually a sign that a majority,

sometimes up to 90%, of the adrenal cortex has been destroyed.

The symptoms of primary adrenal insufficiency correspond to which layers of the adrenal

cortex have been destroyed.

When the zona glomerulosa is destroyed, aldosterone levels fall and that leads to high potassium

levels in the blood, or hyperkalemia, and low sodium levels in the blood, or hyponatremia.

With less sodium around in the blood, water moves out of the blood vessels, which results

in a low blood volume, or hypovolemia.

Finally, fewer protons are lost, meaning more build up in the blood and that results in

an acidosis, and more specifically a metabolic acidosis, since it's caused by the kidneys.

These electrolyte changes and hypovolemia can cause symptoms like cravings for salty

foods with simultaneous nausea and vomiting, fatigue, and dizziness that worsens with standing.

When the zona fasciculata is destroyed, cortisol levels fall and that leads to inadequate glucose

levels during times of stress.

This means that while being chased by a pack of raccoons, instead of feeling ready to sprint

a person might feel weak, tired, and disoriented.

Also, those decreased levels of cortisol causes the pituitary gland to become overactive,

since usually cortisol ihas a negative feedback effect the pituitary gland.

So it ends up producing pro-opiomelanocortin, which is a precursor to adrenocorticotropic

hormone, but it also turns out to be a precursor to melanocyte-stimulating hormone, the hormone

that leads to skin pigment production.

So when your pituitary gland is overactive, it ends up making more melanocyte-stimulating

hormone, resulting in hyperpigmentation, or darkening of the skin, especially in sun-exposed

areas and joints, like the elbows, knees, and knuckles.

In some extreme cases of primary adrenal insufficiency, the zona reticularis can be affected as well,

and androgens levels can fall.

This decrease doesn't affect men much because remember the testes are the major source of

male androgens.

However women can experience a loss of pubic and armpit hair, as well as a decreased sex

drive.

Oftentimes, the slowly progressive chronic symptoms of primary adrenal insufficiency

are missed or ignored until a major stressor, like a serious injury, surgery, or infection,

suddenly causes the symptoms to become really severe.

In other words the body has a sudden increased need for aldosterone and cortisol, and the

failing adrenal cortex simply can't deliver.

This is known as addisonian crisis, or acute primary adrenal insufficiency, and it usually

happens when the majority of the zona glomerulosa and zona fasciculata are destroyed.

It can cause a sudden pain the lower back, abdomen, or legs, with severe vomiting and

diarrhea, followed by dehydration; low blood pressure; and loss of consciousness.

Left untreated, an addisonian crisis can be fatal.

Addisonian crises can also arise from Waterhouse-Friderichsen syndrome, which is when a sudden increase

in blood pressure causes blood vessels in the adrenal cortex to rupture, filling up

the adrenal glands with blood and causing tissue ischemia and adrenal gland failure.

Primary adrenal insufficiency can be diagnosed with an adrenocorticotropic hormone stimulation

test.

During the test, a small amount of synthetic adrenocorticotropic hormone is given, and

the amount of cortisol and aldosterone produced in response is measured, which helps you figure

out how well the adrenal glands are working.

Usually individuals with primary adrenal insufficiency are treated with hormones to make up for the

lack of cortisol, aldosterone, and androgens.

They typically have to be taken for the rest of an individual's life, and stopping the

hormone replacements can lead to Addisonian crisis.

All right, as a quick recap, primary adrenal insufficiency is a failure of the adrenal

cortex - specifically, the zona glomerulosa which causes low aldosterone, as well as the

zona fasciculata which causes low cortisol, and in severe cases, the zona reticularis,

which causes low androgens.

Number two is the yellow house.

And it has Clumsy one of my favorites.

And look at the house.

It looks a little different.

It is a yellow mushroom house. Now, let me just open this.

Spitfire – he is a dragonfly.

Clumsy looks like he hit his head or something.

These figures are so cute.

OK, let's move on to number four.

Number four is the orange house.

And it has Hefty and the bunny.

And the bunny's name is Bucky.

Let me just look at his house.

He has some books and some chairs and some work-out things.

To strengthen his muscles more and more every day.

Here's the bunny.

He glows in the dark!

I'm going to turn off all the lights because this bunny glows.

Here's hefty doing the one arm push-up.

OK, let's move onto number five.

Number five is the purple house.

Come out purple!

Jet Set Radio Future: Cheep Cheap Games Ep 8 - Duration: 5:50.

Welcome back to Cheep Cheap Games, the show where we talk about great games for great prices!

When I was a young lad, I never owned the original Xbox.

Microsoft was new to the console game, so I was a bit hesitant to buy in.

But there was one game that I always wished I could play

and it was actually developed by my lost love, Sega.

And that game was the spectacularly stylish, Jet Set Radio Future!

An inline skating and graffiti hybrid, that is an essential for Xbox owners

and one you can still pick up without spending too much cash.

Let's check it out!

Jet Set Radio Future was released in 2002 as an Xbox exclusive.

This was the second game in the series,

with the original Jet Grind Radio seeing its home on the Sega Dreamcast.

But after Sega bowed out from the console game,

they went 3rd party and brought their IP's to all of their former competitors.

"Future" was a sort of reimagining of the first game, with a similar story, environments and characters,

but there were enough differences and additions to call it a true sequel.

The game follows a gang of in line skaters, called the GG's, as they have turf wars with other rival gangs.

Now, when you think gangs you must be thinking they're some pretty tough customers,

but all the gangs here in Tokyo-To seem to solve their differences by simply having races or trick contests.

It's quite civil really.

The GG's are also caught up in a battle with crooked cops, and a mega corporation

who controls the city and want to destroy freedom of expression.

With a task that big, the gang should have some pretty extraordinary weapons, right?

Actually, they are only armed with cans of spray paint and their sick inline skates!

You have a few characters to choose from at the start of the game but you can unlock

a lot of others who all have different stats as well as their own unique style.

One of the first things you'll notice is that the focus is not really about doing tricks.

In fact each character has a limited amount, especially if you compare it to games like

Aggressive InLine or Tony Hawks Pro Skater.

Your main objectives are typically to tag over rival gangs graffiti with your own,

participate in races or to mimic an opponents trick line.

The controls are really simple with a button to jump, one to trick, and another one to spray your graffiti.

Jumping is especially floaty and it can catch new players by surprise,

but it lets you catch some serious air and cross huge gaps.

When you jump on to a rail you can perform a grind and it will help you get places faster.

And forget about the laws of physics!

Because you can even grind up 90 degree angles.

It's awesome!

Littered through out the stages are cans of spray paint

which are your ammunition for tagging and you can hold up to 30.

If you have more than 10 cans you can do a jet boost to send your character speeding across the streets

Each level will have certain spots you need to tag and you can view them on your map.

Get to that location and spray away to complete a level!

And unlike its's predecessor, here all the levels are interconnected which make it actually

feel like a city, with traffic and people walking around.

Skating through crowds will send people running in every direction, as they shout at the reckless skaters.

Boy, does it remind me of my youth…

There are no longer time limits, which I really like, because now you don't have to rush around

and can explore these large levels at your own pace.

Surprisingly enough, there are even boss battles.

The leader of the cops will often send hordes of minions out to stop you from tagging the town.

Sometimes he'll even personally come to attack you with guns, choppers,

and other types of mechanical weaponry.

All you need to do is spray some paint on the baddies to blind them

and this will somehow make them explode!

Graffiti is a central theme, so what kind of game would this be if it didn't give you cool looking options!

Each level has secret Graffiti souls to collect and this will increase the amount of designs you can tag with.

You can even design your own works of art in the graffiti editor

and then you can spray your work all over Tokyo!

But if you're like me and suck at art, this might not get too much play time.

But for those artists out there it's a nice addition and I'm sure you can make some awesome stuff.

Around the time this game came out a lot of other games were doing

a similar graphical style with cel-shading.

Jet Set Radio Future looks fantastic even 15 years later

and the cel-shading is a perfect fit for the graffiti motif.

I've always really loved this style.

The character design is highly original, and gives the game a futuristic feeling

with the way each character is dressed.

Yup, the gameplay and graphics are down right fun at every level,

but yet they are still eclipsed by the amazing soundtrack.

The music is what ties the whole atmosphere together featuring an eclectic mix of genres.

There's punk, hip hop, J-pop, electronic and funk all thrown into a pot

and cooking up an underground in-line skating stew.

It's so impressive that all this variety can meld together and fit the games tone so well.

Listening to any track in the game will leave it stuck in my head for the next few hours.

What add's to the authenticity of the underground feel, is that you get to know the DJ spinning the tracks.

Professor K is not only the DJ but he also provides you intel on the goings on in Tokyo

via the pirate radio station Jet Set Radio.

Jet Set Radio Future and the original have a small devout following of fans, and it's easy to see how

this game influenced others like Sunset Over Drive, Splatoon, and Hover: Revolt of Gamers.

It's a shame how this never saw another sequel, but it's an inexpensive cult class

you don't want to miss out on!

If you're a fan of outrageous arcade style games that you can pick up and play anytime,

then make room in your collection for Jet Set Radio Future!

Prices currently range from about 15-25 dollars, and it's definitely worth it.

But wait a second, this is Cheep Cheap Games!

We like our games under 10 bucks!

So, if you really wanna save some cash, Jet Set Radio Future came on a split CD with Sega GT racing,

and that edition can be found for well under 10 dollars, and I picked up mine for 3!

bag two bag three a little revit for bag

five bags six seven eight nine and ten

ten bags here also and then these mega

constructs are a lot easier to put

together than Lego so our smaller

children should have what needs your

time to do it see if it shows you this

doesn't look like it's going to show the

other set usually they'll show the other

okay here's the other sense which

already showed you so it shows all of

them here okay awesome let's put this

together and have some fun okay guys

first we're gonna go ahead and take a

look at tie rocks because he looks

totally awesome so you've seen how he

went together so it's not complicated ah

this is very good for our like younger

kids because it's much bigger pieces

than Lego okay and uh good moving parts

us back we'll move here like that you

could ah the shovel here you could have

put together to pick something up like

for instance let's go ahead and pick up

revit so squeeze around rub its body

here and it might be too heavy the whole

yeah

but I good for imagination play then but

you could still open and close it that's

cool he's got arms that spin around his

mouth opens and closes like quite a bit

so you could have it like picking up or

and eating whore and stuff so that is

really cool and then there he is and

then we also have awesome be strong so

he has a spinning blade on the back here

so if you push it hard it'll spin like a

pretty good amount and this moves back

and forth so he could cut stuff up with

his blade moving arms rolling wheels

because both of them they do have wheels

under their tread so they roll along

really nicely you can see his eyes are

on this side where it tie rocks is on

the front his mouth opens all the way

off also so you could have them eating

or or attacking so you could have them

like attack each other is trying to cut

them grab them with displayed lift them

up and flip them over Wow have a lot of

fun then on top of that it comes with

this cool little oasis here you would

have or so you could have them like eat

the or whatever I mean to have tie rocks

or or you could take the or and put it

on his head like a helmet I'm so

hilarious and then it comes with awesome

little rabbit here it comes with this

little oasis which we got here with a

sign of revit making schematic of how

our tie rocks could like attack these

trucks and beat them then we got a nice

little palm tree that goes here so guys

this is a really awesome fun set

and now we're going to have these guys

helped us to identify some awesome

Brosius dinosaurs three

okay our first I know is gonna be a huge

one this is the COS effect bull t-rex

from fat suit poor guy is huge he does

roar his mouth opens all the way up he

could swallow full-size action figures

this video let's go ahead and place this

guy right back here by the gate also

destructs better watch out and here we

got the next awesome title do you know

what this guy is all right he's a Carnot

as women die no this guy is freaking

because when you move his leg he opens

I mean that is freaky looking and he has

got two horns on the front of his head

could find this one is on ebay so like I

actually comes with a cast I don't have

the cast but here you can see his leg

like separate there so it looks like

bone their mouth goes open and closed so

this one is cool lookin too we're going

to go ahead and place this one right

back here with the bull t-rex and

pretend that that is in Swami okay and

who is this little guy wha cause it's

kind of cool this is a magic asaurus and

this is why Safari limited i also have

all proven fucker so you can see it

almost looks like a herbivore but then

when you look how sharp it piqued our

this is definitely a carnivore so I

think we're gonna place this one right

here hiding in the weeds sort of like

the Velociraptor would okay and who is

this little guy like armored back he's

got like a club tail he looks really

cool yes you're right this is this is a

u+ f away so that name is kind of hard

to pronounce by Safari limited we cool

Dino this an herbivore or carnivores you

are correct that is an herbivore we're

going to go ahead and place her over

here and then we have another awesome

Dino this lovable guy is

mark correct that is not a lovable

little guy third cloth who's using

giveaway yeah is a velociraptor this was

made by papal I also did a full review

on that and papal does a great job on

dinosaur models pretty similar papo &

schleich a pretty similar in like

quality and build texture and everything

of these dinos so we're going to go

ahead and place the Velociraptor back

here in the weeds too hopefully it won't

eat our herbivores okay our next Dino is

creaking rolled in shit this is a ramp

or incus and flying so the chorus or you

Published in 1949 "The Intelligent Investor" by Benjamin Graham takes on investing approach

called "value investing", a concept he created from his experience. After losing

most of his capital in 1929 market crash, Graham sought safe and proof investing options.

Here are the most important lessons from The Intelligent Investor:

Never give up and learn from your mistakes Although he lost almost all of his capital,

he didn't quit as most would have. Instead, he learned valuable lessons about risk that

resulted in analyzing risks and valuing them. Investing isn't speculating or gambling

Markets should be looked at as an ally. You should only buy when prices are low and sell

when they are high. Minimizing risk

Graham always implied how important it is to buy investments when it's under its true

value. This is allowing profit when the market revalues or offering protection if the market

goes downwards. The margin of safety

The key point here is buying assets when their value is lower than their true value, and

then later on selling those assets when their value is back to normal or even goes up thus

earning a return from your investment. See beyond the horizon

Keep your eyes on the long-term outcome. There is no way of knowing what markets will do,

markets will fluctuate and always come back to its true value.

Lush How It's Made: Honey Lip Scrub - Duration: 1:26.

- Hi guys, I'm Aura

and today on How It's Made we will make Honey Lip Scrub.

(lively music)

One of the cool things about Honey Lip Scrub

is that it has real Canadian honey

and it will leave your lips soft and plump.

The first step is to get a bunch of jojoba oil,

which will really soften your lips.

Next we'll add our real Canadian honey,

followed by our refreshing peppermint oil.

After we have these ingredients together,

we will quickly mix them up.

Next we add our sugar,

which is a great natural exfoliant.

We give that a quick mix

and then we add our vegan white chocolate.

Once that's mixed

we'll pass it to our production assistants to pack it.

Yamaha CS01 and MR10 - Analog single oscillator keyboard and drum machine - Duration: 6:08.

Hello and welcome to another episode of 8-Bit Keys.

So, recently I was visiting my friend Brandon who has very large collection of vintage professional

keyboards and out of the corner of my eye I saw this.

Now, I thought it was a toy keyboard at first because it definitely has that shape and appearance

to it.

But, actually, this is a professional keyboard.

It's a real analog synthesizer.

So here's a closeup of this thing..

This was produced in 1982.

The controls are all ridiculously simple, but I'm not going to have time to demonstrate

every one of them.

I will mention that it has pro style output for headphone and line-output.

However, inserting anything into either of these jacks will disable the internal speaker,

which is weird..

I would have expected that with the headphone jack, but not the line-out.

Now, at first I was baffled by some of the design.

For example, I did notice that it has these little holes here.

Now, I found out that these are actually for straps.

And the reason that the pitch bend and modulation wheel are there is because this is designed

to be strapped on and played on stage like this, and that way you can access the different

wheels.

Also, the line-output ports are on the side.

Which would make sense, because if you were holding it like this, strapped on, then it

would come out, kind of like a guitar.

So, this is definitely meant to be played on stage.

So, let's power it on and see what we can do with it.

Let's play with the different waveforms.

This is triangle, then sawtooth, then square wave, and then pulse wave.

This switch essentially changes octaves, giving the keyboard a wide range of notes to play.

What I find interesting is that if you use the square wave with no modulation and no

envelope, it sounds exactly like a PC speaker from old MS-DOS games.

OK, so let's add some decay to the sound, you can see how that affects it.

OK, let's play with the pulse wave and try modulating it some.

So, I wanted to put together a little multi-track recording for you so you could hear what you

might be able to do with a keyboard like this.

But, there's no real good way to make any drums on this keyboard, so I also my friend

Brandon's Yamaha drum machine.

Now, this is from the same time period and does kind of match the keyboard.

Let me show you a little bit about this thing.

Alright, so this drum machine is about as basic as I have ever seen.

You can change the sounds of some of the instruments, or the volume of certain instruments.

And you can have it play from some pre-set rhythms.

And that's honestly about it.

I think the sounds are also produced with analog circuitry, there are no PCM samples

here.

Alright, so the song you're about to hear was actually not composed by me.

It was actually composed by Anders Jensen, who does a lot of the music for my channel.

But, he actually composed this upon my request as a theme music for the upcoming game that

I'm producing called Planet X2.

So, eventually you're going to hear this same music comming out of a Commodore 64.

But for the moment, you can hear it coming out of this Yamaha.

So let's take this thing apart and see what secrets it hides.

There are just 6 screws on the back.

So this is technically the back side of the board, but one thing that jumps out at me

is how they labelled everything on here.

It's almost like they were inviting modders and hackers to come in here and change things,

which is pretty cool!

So let's take out the board and look at the top.

There's not a lot here other than analog components and this one large IC, but I haven't

been able to find any information on this chip.

Sesame Street and learning kindness and generosity from the incomparable Mr. Rogers.

Public television also gave me my first exposure to art, when Sister Wendy took me on tours

of museums I couldn't travel to, and Art21 is an amazing series that offers a window

into the lives of working artists, and which helped sparked my own interest in art as a

career.

The Corporation for Public Broadcasting, which provides critical financial support to local

public television and radio stations, currently receives $445 million dollars from the federal

government annually.

That is definitely a lot of money, but to put it in context this costs just a $1.35

per person per year.

That $1.35 per year gives you, and the people you share your world with, an enormous amount

of non-profit educational content.

You can really of public television is America's largest classroom.

68% of all kids ages 2-8 watched their local public television station last year.

If--like me--you don't want this invaluable public service to go away, there are a number

of things you can do:

You can contact your local congressperson and tell them why you love your local station

and PBS.

Or show up at a town hall.

Or make a YouTube video and share it with them.

You can also share the message with your friends.

Post on social media about what PBS and your local station mean to you with the hashtag

CS:GO - Useful Anti-Rush Smokes on Cobblestone - Duration: 2:28.

Hey guys, salut mes amis och hallå gubbar.

Next map in the series, Cbble.

Let's start with the smokes for B. The two ways I have is, in this corner, aim here

for 128 tick and here for matchmaking.

Run just a little bit and jump-throw.

This will land before B entrance.

The terrorist will have no way of passing this smoke before it lands.

Second way, and the safest is in this corner, aim at the top of the hole in the wall, and

throw the smoke when your crosshair meets the bottom.

This smoke needs to be thrown fast.

Next smoke is for drop, go to this wood plank, aim at the right of this wood plank, move

your crosshair up halfway, and throw.

This will create a one-way if you boost yourself up.

It's recommended to first throw an incendiary, then the smoke, then boost.

Next smoke is for middle doors, line yourself up like this, and aim to the left of the black

lines like this.

Not only does it smoke off doors, but it creates a descent one-way, be careful

not to stand too much out in the open or you might get pre-fired.

Last smoke is for A long or catwalk, line yourself up like this and

throw here.

This not only smokes the passage but it creates a convenient one-way as well.
