Volvulus - causes, symptoms, diagnosis, treatment, pathology - Duration: 6:05.
The term volvulus actually comes from the Latin word volvere, which means "to roll".
So a volvulus is an obstruction caused by a loop of the intestine that rolls or twists
around itself and its surrounding mesentery, which is the tissue that attaches the intestine
to the back wall of the abdomen.
The three most common types of volvulus are a sigmoid volvulus, which happens in the the
last part of the large intestine, leading to the rectum; a cecal volvulus, which happens
in the beginning of the large intestine, and a midgut volvulus, which happens in the small
intestine.
Now, a sigmoid volvulus is the most common type of volvulus, and it can happen in a variety
of settings.
One classic one being pregnancy, because the growing fetus can cause displacement and twisting
of the colon.
It can also develop, though, in middle-aged and elderly individuals.
This can sometimes happen as a result of chronic constipation, where a big load of stool can
act like a pivot point around which the rest of the colon can twist.
Hirschsprung disease, a disease of the large intestine that causes severe constipation
or intestinal obstruction, therefore raises the risk for developing sigmoid volvulus.
In addition, there are also abdominal adhesions, where internal scar tissue creates a physical
attachment between two parts of the abdomen, which again serves as a pivot point around
which the colon can twist.
A cecal volvulus is usually found in young adults, and usually happens in individuals
who didn't develop their abdominal mesentery normally during fetal development.
Since some mesentery contacts may be missing in these individuals, the colon can flop around
freely and any large object—like a baby in pregnancy or a load of stool in someone
constipated—can act as a pivot point in the cecum and cause the colon to twist.
Midgut volvulus is most commonly found in babies and small children and is the result
of abnormal intestinal development in fetuses.
In normal fetal development, the digestive tract starts as a straight tube from the stomach
to the rectum.
For a little while, a part of the intestine protrudes from the abdomen into the umbilical
cord.
Once the fetus reaches around 10 weeks, though, the intestine pulls back out of the umbilical
cord, and returns to the abdominal cavity and makes two turns, so that it is no longer
a straight tube.
Malrotation happens when the cecum and appendix, which are normally found in the lower right
side of the abdomen, stay in the upper right side.
Babies with malrotation can later develop twisting of the small intestine, which is
a midgut volvulus.
In any type of volvulus, a portion of the intestines becomes twisted and it pinches
the lumen shut and results in bowel obstruction, which prevents the normal passage of digested
food and water.
Sometimes, the mesentery can become so tightly twisted that blood flow to that part of the
intestine is cut off, which leads to infarction, or death of the intestinal wall.
This can cause a variety of symptoms ranging from mild bloating and constipation to severe
pain and bloody stools.
In fact, bowel infarction can also cause the intestinal wall to break down and allow the
bacteria in the gut to move into the body, which causes sepsis and the potential for
cardiovascular collapse.
Abdominal x-rays are usually used to diagnose volvulus.
These x-rays show the shape of the volvulus, which can look like a bent inner tube or a
coffee bean.
A barium enema can also be used to show a bird's beak shape, enlarged at one end and
tapered at the other end, which is the point where the bowel is twisted.
A sigmoid volvulus is usually treated with sigmoidoscopy, an examination of the sigmoid
colon using a set of flexible tubes inserted through the anus.
If the tissues look normal and pink, the tubes untwist and decompress the colon to relieve
pressure.
A similar procedure that goes a bit further in, called a colonoscopy, examines the entire
colon and can occasionally be used to resolve a cecal volvulus.
But a midgut volvulus can't be treated with colonoscopy, since it's even further back
in the small intestine.
Normally surgery is recommended within two days of receiving treatment, or, in some cases,
surgery should be performed immediately, like if the bowel is severely twisted or if the
blood supply is cut off.
The normal surgical procedure involves untwisting the intestine, if it hasn't been corrected
yet, and attaching the intestine up against the abdominal wall to prevent it from twisting
in the future.
In severe cases, such as infarction, pieces of the intestine have to be removed, a process
called bowel resection, before the intestine is reattached.
Aright, as a quick recap: a volvulus is a twist in the intestine and surrounding mesentery,
and it's most commonly found in the sigmoid of middle-aged and older adults, the cecum
of young adults, and the midgut, or small intestine, of babies.
Volvulus results in bowel obstruction and can sometimes cause infarction, or death,
of the bowel tissues.
Volvulus is diagnosed using x-rays and is usually treated with surgery to remove part
of the bowel and attach the bowel up against the intestinal wall.

Hi!
Today I would like to tell you something no one talks about on the Internet,
or at least almost no one.
As you can see in the title before clicking on the video,
because that's what normal people do.
Well I'm going to talk about that sort of war opposing Facebook to Snapchat.
What you need to know first is that it's an one-sided war,
because only Facebook is participating, Snapchat doesn't really care.
First I'm going to contextualize.
Facebook was founded in 2004 by Mark Zuckerberg and is the most famous and used social media
in the world with more than a billion active users everyday.
In 2012 they purchase Instagram for the price of a billion dollars,
you'll see, this info will be useful for later in my explanation.
On the other hand, there is Snapchat, an app created in 2011 by Evan Spiegel,
who is today the youngest billionaire in the world,
replacing Mark Zuckerberg, very funny.
Really funny.
And the Snapchat app is developed by Snap Inc,
founded in September 2016, the parent company which comes ahead,
exactly like Alphabet and Google.
This is where it's becoming interesting.
In 2012, Facebook launch Poke, an app which is a near-exact copy of Snapchat
precisely to compete with Snapchat.
Of course it doesn't take off and compensate that failure,
Facebook tries to buy Snapchat for $3B.
Not that much...
Of course Snapchat declines and from there Facebook starts the hostilities .
It's insane, insane, from there... woooh.
Ouch aaah, I punched the wall.
And then it's the beginning of countless attempts from Facebook to get to Snapchat's level.
Spoiler alert: they don't succeed.
Ah shit, I'm bleeding...
And it continues in 2014 with this time an app called Slingshot.
Still the same concept, although slightly different this time,
you need to send a picture to see one,
it's complicated, not practical, one again a Snapchat copy and another failure.
We're in 2014 and there is ONCE AGAIN an app which allows
to send to send destructibles items.
This time it's from Instagram and it's called Bolt, it's the same concept as Snapchat
but way less better than Snapchat.
An umpteenth attempts which is going to end in the depths of Internet.
We suppose that they understood their lesson at Facebook,
trying to create an app too similar to Snapchat, it's going nowhere.
So Zuck has an idea.
They are now going to only take pieces of Snapchat.
And they are going to add them in Facebook products.
So in 2015 appear the Disappearing Messages,
a feature only tested in France.
It's the outbreak, absolutely everyone is using them.
By mistake...
...but everyone is using them.
You perhaps remember, you had the ability to enable them
to send messages which an hour later wouldn't exist anymore.
I know a lot of people who activated this option,
I don't know anyone who activated it on purpose.
Today this feature isn't available because to send those kind of messages
you just have to use the Snapchat chat.
Becomes it's where the idea come from.
Way more practical.
I did a Snapchat story about the upcoming subject, those who follow me, probably saw it.

Early 2016, Facebook purchased the app MSQRD,
developed by a Belorussian startup, Masquerade.
Exactly the same as the Lenses on Snapchat.
How is that? You don't know what the Lenses are?
Of course you do, those Augmented Reality animations that y'all are calling "filters" or "effects"
and which aren't funny anymore since the day we puked rainbows.
Yep... since the first day.
MSQRD still exists and it's an alone app
not very famous and useless, mainly when you know that you can do the same on Snapchat.
Directly IN Snapchat.
Next thing is heavy, originality speaking with the Messenger Codes.
You maybe missed it, I didn't,
when you go on your profile for the Messenger app,
you can see that code which oddly reminds us of Snapchat's snapcodes.
So yes, I know QR Codes weren't invented by Snapchat,
but the concept to add someone on social media using a flashcode comes from them.
Contrary of snapcodes that I've seen used a lot,
I never saw someone use that.
Actually, I don't even know if a lot of people are aware of that feature.

Here's the best thing.
Facebook from now on forbids you to have a snapcode as profile picture.

When you can't get to the level of your competitors,
why not prevent them from spreading, easier right?

so when I had it as profile picture on my Facebook page, it never got noticed.

Because Facebook is a mainstream social media for the 18-25 years old
and that the best social media for this age rage is Snapchat,
this time they decided to attack differently.

It's an Instagram update.
The arrival of Instagram... stories.
Without a damn concern,
they stoled not only Snapchat's biggest feature's name,
but also stoled the entire fucking idea.
For once, everyone heard about that.
I mean it's so big for an intellectual property theft that we couldn't miss it.
I'm still waiting for Snapchat to drag those thieves of Instagram and Facebook to court,
for that unscrupulous theft,
so they'll get back the billion of dollars they deserve.
Recently Instagram added new features to their stories
that aren't on Snapchat.
And for the first time in this tale,
Snapchat answered to this theft by also adding those features to their stories.
And still today, Instagram is continuously stealing
Snapchat ideas to put them in their stories.
At Instagram HQ, they don't do meetings to now what improvements they can do
for their stories, no no!
They do meetings to know what improvements thay can steal from Snapchat.
If you ask me, I'm 100% team Snapchat stories.
Their stories Instagram, they can make Mark Zuckerberg eat them.
I'm in constant boycott of Instagram stories and I don't hesitate to criticize those who watch some.
Or those who post some.
By the way, there's been a new Snapchat update recently
which was put online the day before I shot this video.

It's made as an anniversary.
So, a new version, the 10.0.0.0 version for iOS, a redesign of the app.
With this new version by the way, they announce that they're going to fight fake news,
mostly on Discover, which seems necessary
when we know that in only 10 days governing, Donald Trump and his administration
threw to our faces a lot of "Alternative Facts".

Facebook trying desperately to copy Snapchat, in vain
since Snapchat refused to be bought by the Palo Alto firm.
While the Venice company, has more a strategy which consists in ignoring Facebook's
debacle and let them try everything to get to Snapchat's level.
Snap Inc know that they are above them and don't belittle themselves to Facebook level
trying to simply steal others' ideas.

LEGO Batman Movie 2017 Summer sets pictures! - Duration: 1:50.
Hello just2good here,
and the Hollywood Reporter
has revealed four out of the five LEGO Batman Movie sets for Summer 2017.
The Batwing retails for $90 United States Dollars,
and has Harley Quinn, Batman, and Robin.
The Two Face Double Demolition retails for 60 bucks
and has Two Face, two GCPD officers, and Batman.
Bane's Toxic Truck retails for 50 bucks
and has Bane, Mutant Leader, and Batman.
And, Scarecrow's Gyro Copter retails for 15 bucks
and has Scarecrow and Batman.
Missing is the fifth set which has the Wicked Witch
and such, possibly to avoid spoilers.
Also missing is Polka Dot Man,
who is supposed to come in a set...
Honestly, I'm disappointed in most of these.
I think the Scarecrow set is
the only one that lives up to my expectations...
I really like how it includes more attachments for the Gotham Energy Facillity.
The other three just don't feel like
they're worth the price.
Okay, well,
I mean the Bane set looks decent for 50 bucks.
I don't like the car but at least it feels big enough
to warrant the price, and you get two new minifigures.
The Two Face set looks so small for 60 dollars,
and I wish we would've got more unique villains in this one besides Two Face.
The two GCPD officers are nice
but a new character would've been more interesting.
The build isn't bad but does not look like it's worth the 60 bucks.
Alas, the Batwing is big enough for $90 and looks great,
but I just wish it had more minifigures than three characters
we already have and one exclusive varient.
This movie is full of colorful and crazy characters,
it makes sense for the sets to have a lot of different weird characters,
but these summer sets lack in that department.
Anyways, like I said,
there's still one set to be revealed.
Oh, and the article revealed the brick headz
will be $10 each which are great prices!

signed presidential order to build border wall with Mexico
Obviously Mexico doesn't like that one bit
I don't know what exactly said former Mexico's president Vicente Fox
who earlier said Mexico should not pay for that f***n wall
But liberal media report that it is at the Americans
who will pay for the wall (card insert: Poland's public broadcaster TVP - "Americans will pay for the wall:"

media don't say that it is Mexico to pay
America will pay and be reimbursed later
Cost of the wall varies
Donald Trump in primaries mentioned $10B
now the media report $20 - 40 B plus even - I've sen such estimates
the cost in the media reports grows by leaps and bounds
while it is estimated that an average cost of 1 mile of such border fence
should cost approx. $10M a mile
a pararel fence would cost double that
so media estimates don't add up
The cost will be much lower
and in my opinion knowing Donald Trump's business attitude
the cost will be even lower than that
And should it be an undertaking
involving a concrete structure
this would be the largest construction project
in contemporary US history
since Hoover Dam
which stood for an engineering symbol of America
That's at least what's been said about the structure to be erected
President Trump also ordered an increase in border patrols by 5000
and the end to catch and release policy
Thus far those caught on the border illegally
had their fingerprints taken
but then were released - but here in America
they skipped deportation hearings
and disappeared within the country
Now the president says - enough of that -
you will be caught and released - but on the other side of the border
back where you came from - Mexico
President Trump also ordered the fight with sanctuary cities
There are dozens of such cities in the US
which implement very liberal policies towards illegal immigrants
Large sites such as New York , Chicago LA or SF
which provide protection to illegal immigrants
by providing them with ID cards and welfare money
thus protecting illegal immigrants
But their problem is
they got huge budget deficits
and need to be supported with federal money
they receive federal grants to function
thus we will be able to increase leverage
to comply with government policies
President Trump also plans to introduce
another executive order which would suspend
visa travel from Musilm countries
which are prone to send us all sorts of terrorists
Presidential orders multiply
but these are presidential orders
which do not create new laws
but implement existing ones
Now on Friday at the Pentagon
president Trump signed an executive order
to modernize US military forces
which under pre. Obama shrunk dangerously
Under pres Trump we will see new military planes
ships, weapons and technologies
and the modernization on nuclear arsenal of the US
all this signed last Friday wt the Pentagon
and with a historic visit
UK's PM Theresa May arrived to Washington DC
who met with President D. Trump at the Oval Office
A very short meeting and then a press conference even shorter
No major surprises in the meeting
except for one faux pas

you don't quote what the other party said in those meetings
especially someone like D. Trump who earlier
was building his leverage against NATO
saying he will demand payments from NATO countries which fall below 2% of GDP
and called NATO obsolete - building his leverage for future negotiations
and here Theres May says - You yourself told me you supported them 100%
I looked at Donald Trump
and I don't think he liked it
there was a cold Bethlehem steel in his eyes
Betlehem used to be big on steel here in the US
And another interesting issue in the meeting
It's when President Trump and Theresa ay enter the Oval Office

what I hope you know -
who - if Germany won II World War
would have hanged from scaffold at Nuremberg for war crimes

and other German cities
as he could n'twin the war otherwise - so he bombed civilians

effectively eliminated from public knowledge

MR May's visit comes almost to the day

after a potion calling for such ban

but he aimed Mr Trump;s ex- wife statement
alleging that her ex then husband
used to lie in bed and read works of Adolf Hitler at night
I don't believe that someone who
allegedly reads Hitler's works at night
and is of German ancestry like MR Trump
doesn't know true history of Germany's nation
I don't believe it
He knows it
and when they were standing at the Oval office
Donald Trump said- pointing ti the statue of Winston Churchill

and then - in so many ways
Our stupid media = pardon my words - but that's what they are
reported he said that about Churchill - no he said that about the bust
There're two Churchill's busts at the White House
That one was the original coming back from 1960's an LBJ
but then he added - in so many ways
and he meant what Churchill did to German people
in my opinion
I believe Mr Trump has respect and sympathy for German nation
but has little sympathy for Chancellor Merkel
and best example for that
is Theresa Mays first foreign visit to the WH - not Chancellor Merkel's
after all a huge superpower in Europe
I believe we are watching new US foreign policy taking shape
Mr Trump will bet on UK
Why? - He is playing a so called 'classic triad'
When you negotiate sth one on one - it's usually tough
But when a new element emerges - new business, entity, a new state
in that model of politics
it's easier to negotiate - as you can play them against each other
That's exactly what president Trump is doing
When UK emerged after Brexit as a separate entity
separated from EU - he immediately thought
I want to bet on UK
I want to have Mrs May here - not Chancellor Merkel
America thus far treated transatlantic relations linearly
that is - America
then Europe treated as monolith
and Russia
Now under Mr Trump - it's different model
There's America - UK
then the European monolith cracking politically from within
and then there's Russia
So now Mr Trumps; reaching out to Putins Russia over EU's head
and Chancellor Merkel probably - can't sleep well at night cause of that
That's why she was seeking meetings with Obama in Berlin
talked about TPP and TPIP let's do it quickly quickly shnell shnel
as she knew if Trump arrives - he will go over her head
And that's why the role of Poland is now so important in all this
Poland is a wedge between Germany and Russia
so Poland is in ideal position
to join the union which is forming
between UK - the oldest union in the world -
and second oldest - the American union
if Poland manage to engage that formation
snd become the balancing act between
Russai and the European monolith of France and Germany
Polans could be in an ideal position
especially so -
that there are 800 000 Poles in UK- according to some estimates
in my opijnon it's over a million
here in US - over 10 million Poles lives here
an ideal position to make politics
in a new political situation
which was created by election of President Donald Trump
