Disseminated intravascular coagulation, or DIC, describes a situation in which the process
of hemostasis, which is when after blood vessel wall injury, liquid blood rapidly becomes
a gel, called coagulation or clotting, starts to run out of control.
When this happens, lots and lots of blood clots start to form in blood vessels serving
various organs, leading to organ ischemia.
DIC, though, is also called a consumption coagulopathy, because all this clotting consumes
platelets and clotting factors.
Without enough platelets circulating in the blood, other parts of the body begin to bleed
with even the slightest damage to the blood vessel walls.
So paradoxically, patients have too much and too little clotting.
Normally, after a cut and damage to the endothelium, or inner lining of blood vessel walls, there's
an immediate vasoconstriction or narrowing of the blood vessel which limits the amount
of blood flow.
After that, some platelets adhere to the damaged vessel wall, and become activated and then
recruit additional platelets to form a plug.
The formation of the platelet plug is called primary hemostasis.
After that, the coagulation cascade is activated.
First off in the blood there's a set of clotting factors, most of which are proteins
synthesized by the liver, and usually these are inactive and just floating around in the
blood.
The coagulation cascade starts when one of these proteins gets proteolytically cleaved.
This active protein then proteolytically cleaves and activates the next clotting factor, and
so on.
This cascade has a huge degree of amplification and takes only a few minutes from injury to
clot formation.
The final step is activation of the protein fibrinogen to fibrin, which deposits and polymerizes
to form a mesh around the platelets.
So these steps leading up to fibrin reinforcement of the platelet plug make up the process called
secondary hemostasis and results in a hard clot at the site of the injury.
Now, as soon as the clot is formed, the body is also initiating pathways to break down
the clot so that it doesn't get bigger than it needs to be and dissolves when it's not
needed anymore—a process called fibrinolysis.
And this process produces fibrin degradation products.
Normally, the formation of new clots and the process of fibrinolysis are in a steady balance.
Now, in serious medical conditions, like for example sepsis, malignancy, serious trauma,
obstetric complications, or intravascular hemolysis like you might see with blood type
incompatibility, there can be a release of a procoagulant that tips the scales in favor
of clot formation.
Procoagulants could be proteins like tissue factor or bacterial components like lipopolysaccharide,
or could even be the presence of enzymes that help to proteolytically cleave and activate
clotting factors.
Whatever the cause is, in response the coagulation pathway goes into overdrive, resulting in
widespread clot formation which plugs up medium and small blood vessels which leads to ischemia,
necrosis, and eventually organ damage.
The kidneys, liver, lungs and brain are particularly susceptible.
The massive formation of clots throughout the body depletes the supply of platelets
and clotting factors.
To make things worse, as the clots are broken down through fibrinolysis, fibrin degradation
products are released into the circulation and these interfere with platelet aggregation
and clot formation, making hemostasis even more difficult.
And the result is a paradox, right?
On one side you have thrombosis, which is when clots obstruct the vessel, and at the
same time you have trouble forming clots, which leads to bleeding.
Laboratory findings in disseminated intravascular coagulation include decreased platelets and
decreased fibrinogen.
There is also a prolonged prothrombin (or PT) time, as well as prolonged partial thromboplastin
time (or PTT), both of which reflect having lower levels of circulating coagulation factors.
In addition to lower levels of specific clotting factors, there are elevated levels of D-dimer,
which is a fibrin degradation product that is produced when fibrin clots are broken down.
Now, it's worth mentioning that in some cases, disseminated intravascular coagulation
can be a more chronic process, like in individuals with certain solid tumors and large aortic
aneurysms, and in those situations, there may be physiologic compensation making the
lab results look relatively normal.
In either acute or chronic DIC, the treatment focuses on treating the underlying cause,
because that's what drives the activation of the coagulation cascade.
In addition, the goals are to support the various organs using supportive measures like
ventilator support, hemodynamic support, and transfusions if they're needed.
Alright, as a quick recap, DIC happens when the balance between forming new clots and
breaking down clots is tipped in favor of clots, which leads to widespread clotting
and organ ischemia, while at the same time depletes clotting factors, paradoxically leading
to bleeding.

Hello. Welcome to a new episode.
Today's topic is Network Analysis part 2.
Examples of the Networks & Communities.
Today, we are continuing with the same topic as the last time.
We will try to build adjacency matrix from the raw data.
Then, we will calculate the modularity of the network and detect the community structure.
We will compare the results with those from the k-means clustering method.
In order to fully understand today's video, please watch the part 1 of the network analysis.
Also, it is recommended the episode on the k-means clustering.
A quick recap from the previous episode.
A network is composed of nodes and edges.
The nodes are connected by the edges.
And, the closely related nodes may form communities or clusters, as we can see here.
A network can be represented mathematically by a matrix called adjacency matrix.
This is a symmetric matrix of 1s and 0s.
The 1s mean close relations.
Here, let's compare the k-means clustering with the network analysis.
They are both unsupervised learning methods.
There is a difference in the terminology.
However, both "clusters" and "communities" have similar meanings.
The difference between these two methods is that,
For the k-means, the number of clusters is given by the user,
while for the networks, the number of communities is detected by the algorithm itself.
OK. Let's see some examples.
We have the data for the example in this Excel file named "Example_Network Analysis.xlsx".
This is the Iris flower data by Ronald Fisher published in 1936.
This data set has 4 variables X1~X4 which correspond to the different measures of the Iris flower.
Originally, this data set also has a dependent variable Y that corresponds to the Species label.
There are 3 different species with 50 observations (or rows) in each, with 150 observations (or rows) in total.
Now, attention please!
I will delete the Y column, so that we are left only with the features X1 ~ X4.
So the question we are asking now is: How much can we retrieve about
the original classification structure that was contained in the Y variable?
To answer this question, let's first perform the modularity maximization.
We go to the PrimaXL tab and then select Network.
Here, for the data range we select the cell range of the variables X1~X4.
Here, by default the top 20% of the cosine similarities are converted into the edges.
Then, we let the total number of communities go from 2 to 5.
The "seed" and "iterate" are parameters for the optimization method.
We can just leave them as they are.
Finally, we specify the output location.
Now, we press the [Run] button.
Great, we have the result!
As we specified, the number of the communities goes from 2 to 5
and we have the corresponding modularity values.
The maximum modularity is detected for the number of communities equal to 3.
It seems that the modularity remains the same for the number of communities equal to 3 and beyond.
Now that we know that the maximum modularity case has 3 communities,
let's bring the lower limit and the upper limit together to 3.
When these two limits coincide, we can calculate the community structure.
So, we check on this box.
Also, we check on this box to output the adjacency matrix.
We check on this box to stream the output to a new sheet.
As usual, we press the [Run] button.
In this new sheet we have the adjacency matrix which is a symmetric matrix of 1s and 0s.
We select this sheet named "result".
We have the modularity, the community structure and a summary.
Remember that originally the labels were 0, 1 and 2.
Here, the labels are 1, 2 and 3.
However, also remember that the labels exist in order to distinguish the communities from each other.
So, they can be any integer value as long as they serve the intended purpose.
Now, let's interpret the result.
Let's compare the results of the k-means and the network.
Remember that we used exactly the same data set.
For the k-means, we retrieved the original clusters with 89.3% accuracy.
And, for the network, the retrieval rate was 97.3%.
I wouldn't jump into the conclusion and claim that the network analysis is superior.
But, for this particular data set, the network performed better.
What is important is that we are able to say the optimal number of communities should be 3.
Finally, a summary.
Today, we used the network analysis as an unsupervised learning method.
It turned out to be quite an effective clustering method.
The network analysis can be suitable as an exploratory data analysis method
when the number of communities or clusters is not known.

Behind the scenes, high ranking Air Force officers are soberly concerned about UFOs.
But through official secrecy and ridicule, many citizens are led to believe the unknown

Theodor C. Loder III, Phd, Professor Emeritus of Earth Sciences, University of New Hampshire,

various encounter with UFOs.

We wanted to show you a traditional Scottish Breakfast
but it wasn't what we expected,
We bit in a turistic trap.
The suposed Scottish breakfast was at the end
and English breakfast, they differ in
that the Scottish breakfast has also Haggies and Black pudding
which is a typical dish and our breakfast didn't have it.
So if you want to try a tradictional Scottish breakfast never come to that place.

The National Portrait Gallery is housed in one of the most remarkable buildings in Edinburgh,

The museum houses 28 paintings from the Bridgewater Collection,
deposited since 1945 by the Dukes of Sutherland and considered collectively as the most
important private collection that can be seen on loan in any museum in the world.

I've actually run into a bunch of other
youtubers, which hopefully we'll get some
time with as well. Right now I'm over
here at Franklin Armory. I have some
history with Franklin Armory in that I
bought a number of things from them that
they produce. They're a great company. They
make some great stuff. Sun was kind
enough to meet with me and show me some
of the things that they have planned for
2017. So what is that you have in your hand.
So this is our latest model for F17.
It's actually the world's first piston
driven rimfire. OK wait a minute. That's a
piston-driven rimfire. Yep. That's pretty
awesome.
It's the only one in the world
that has a piston. It does shoot a 17
Winchester super mag. Which rocks at what?
3,000 feet per second, so thing flies straight!
So wait a minute...
because it's a rimfire, would that be California
legal? It is California legal so you can
have a detachable magazine.
You know I do come from the people's
socialist republic of California, right?
Yeah. I just came out here so I could be in
the United States and Nevada for a
little while... Arizona for little awhile and move
around. So you can have a detachable
magazine. You can have a real mag release.
You can have the collapsible buttstock.
You can have the flash hider. That's very
cool.
How does this compare
ballistically to a 556? This actually compares
very, very close to a 556. It is a
20 grain bullet shooting at 3,000 feet
per second. A little lighter bullet.
a little lighter bullet. But really
screaming fast. But really screaming.
Really flat shooting to about 200
yards. How accurate would you
say this rifle is. I'd say it's accurate
sub-MOA. OK, for boneheaded guys
like me, sub-MOA would mean
you're going to have a group of less
than an inch at a hundred years. Yes, that's
correct.
Cause I'm just a GunGuy. What do I
know. I don't know any of this stuff. So that's going to be a
sub-MOA gun at a hundred yards. That rocks.
Are you the only guys that make one. We are
the only ones that make an AR platform in
the 17 Winchester super mag.
I'm going to build you up a little bit
because I've got to tell you, one of the
things I love about your company is you
guys just think out of the box. you make
some really cool stuff.
What else have you got planned for 2017?
You got something else?
Well, we have two other platforms. We
actually are sending binary triggers in
different platforms. Which
unfortunately, I don't get to take one
home with me.
Yeah. But that's ok. that's ok.
You've got stuff that I can't
touch in California, but if we worked
hard and got those laws change maybe we
could get. Binary
triggers would be among them.
Do you have any other rifles that you
want to talk about? Yeah, we do have the
new binary trigger for the HK. So it
will fit into an HK 91 or an
MP5 style platform. That
would be very, very neet. So we're hoping to
get that out in a few months here.
So, do I get to take this home with me?
Um... No way! He's not letting go.

but this one actually has a binary
firing system in it and we are actually
selling this model with a binary firing
system. Rats! Not in California. That's right,

Yep. What do they call it? It's called the
CSW. In California they call it a rifle.

Yeah! Okay! So, it's a weird thing. It doesn't have
a stock and you can have all the
weird features that California doesn't
on your gun. Yeah, because it doesn't have a stock.
The only problem is I've gotta carry tripod around with
everywhere I go, it really, really is cool.
How does the trigger work on that one. The
trigger it has linked in, you've got two
triggers up here. You link it to your
standard AR trigger and any AR platform
can actually drop into this thing. And you
just pull the triggers and away you
go and it rocks.
I don't have one of those, but
you saw the pictures of it as we were
talking about it and one of these days
I'm gonna get a hold of one of those and I'm
gonna do a video on it. Even if I have to go
all the way out to visit these guys to shoot it
where they. Because that thing is

can find it. Because this may be a
solution for those of us that would like
to have a fully functional AR platform
in a state like California where
rimfires
are not affected by the law as long as
we're talking about rifles. My
understanding is that rimfire pistols...
then they're going to start regulating
those. If you've got an AR pistol or
rimfire chambering. This thing
rocks. Very similar ballistics to a 556 or 223
and yet totally California legal. You
know the other thing I like about it is
the ammo... 223 ammo is pretty light, but
this is really light. Oh yeah. That brings up
a question: on the price of the ammo, is it
roughly similar? It is roughly
similarly. You can get it a little bit
cheaper than a 556, but a little
bit. How available are the mags?
The mags are very available. We have them on
our website. You can always get them
through our website. This is
going to sound like a stupid questions because I
don't know. I'd have to check. I think
that California, even with the rimfire's,
are limited to a 10 round mag, aren't they? Yes, they are.
You're still going to be down to a 10
round mag but, nevertheless you can you
can change your mag fast (snap, shap) and
you don't have to fiddle with it and you can keep all
your features. I like the solution a lot.

A STATEMENT ON THE BUTLER AREA
SCHOOL DISTRICT'S WEBSITE SAYS
IT WAS FOUND ONLY IN THE
UNTREATED WELL WATER AND THAT
THE WATER UNDERGOES DI
DISINFECTION IN THE SCHOOL.
FURTHER TESTING WILL BE DONE
AND THEY'LL TRY TO FIGURE OUT
HOW IT GOT INTO THE WELL IN THE
FIRST PLACE.
IN THE MEANTIME, STUDENTS ARE
OUT OF SCHOOL FOR THE REST OF
THE WEEK.
PARENTS, THOUGH FRUSTRATED,
PLEAD THEIR CHILD'S SAFETY IS
COMING FIRST.
