Waching daily Sep 12 2018

Hey guys, welcome to our channel

Today we're challenging ourselves

to make an arepa ice cream sandwich

- Arepa ice cream?

- What?

- Arepa can't be a dessert?

- Never!

- I am from Venezuela

and I love arepas

It was my daily bread

Before corruption took place

But what's an arepa?

An arepa basically is like

a corn based bread

It's very popular in Venezuela, Colombia and Panama

and basically all of Latin America

because we all love corn

It just has different names

I have seen so many videos

featuring weird ice cream flavors on Youtube

That I decided

Why not?

We have no idea how this is going to turn out

So please don't follow the recipe until the end

Of course I cannot do this crazy thing alone

So I had to call the whitest person I know

My friend and food producer, Tess

- Hi!

My name is Tess Middlebrook

And I'm a food producer

and comedian

And I've been cooking for the last 13 years

I'm not going to tell you my age

Just know it's a long time

I am the whitest person, actually

If the sun hits me in the right angle

You can see my heart beating

So I actually did make arepas last night

to test them out

They're very delicious

And I'm here to

have fun with arepas!

Arepa, arepa...

It reminds me of a west side story

- No we cannot use that

- I can't believe you want to make arepa ice cream

- Right

What I had in mind for this project is

How can we make an ice cream out of an arepa?

And how can we use the arepa

as a sandwich for an ice cream?

Because the arepa is so flexible

We put so many things inside

I wonder if we can put

like cheese ice cream

Avocado ice cream

- I know that cheese is a common pairing in arepas

- Right

- Maybe we make an arepa ice cream

with cheese

and a little bit of corn

Then make an avocado ice cream

And then we use 2 arepas as a sandwich?

- I think this might work

- Then let's try it girl!

Double up!

- What do we need to know, what's the first step?

- Right...

First we have to infuse the arepas

into the ice cream base

- Alright

And to get some inspiration, we got some Venezuelan rum!

- Oh wow, okay

It's 9:00 AM and she's getting busy!

And we have these beautiful Turkish coffee mugs!

- Chin chin! - Chin chin!

It's good though!

- It's really good, yeah

- It's Venezuelan rum

- It tastes like I feel!

- There are two ways to make arepas

You can either use the flour

Or you can make them from scratch with sweet corn

Right now we're going to use the flour

Because guys, we're making two ice creams

and an arepa sandwich

Ain't nobody got time for that!

- We're going to do 2 cups of...

- Why did we have that rum?

CACIQUE

- 2 cups of flour, yeah?

- You can make the arepas eithe with water or milk

We're goign to make them with milk

I love how my family cooks

Because they're just eye balling everything

When you ask for a recipe it's like 'Until you see!'

- How do you say that in Spanish?

- Hasta que lo veas

- Hasta que lo veas

- Come stai? No, that's Italian

- That's Italian, Tess

2 and a half cups of milk

Time to get dirty!

- You're going to do it with your hands?

- Yeah!

How were you thinking of doing it? With forks?

- Go ahead, sure

- We're going to a add a little sugar

Which we should have added already

I forgot how to make arepas!

- So you add a little sugar to yours

I didn't know that

- And salt

- It would have been good to know when I shot my vlog

So we need a little bit of butter

- We don't have

- Hold on, let me just go find it inside this ceramic dish

¿Okay, ready?

- Ready

- Oh my Gosh! You formed it!

- Girl! I've been making arepas for 21 years!

The thing with arepas is that you shouldn't let it dry

- We have some butter on the pan there

I really love this corn flour

Look! Nothing on your hand

I don't what she's doing

Fried arepa

- Fried chicken

- You guys gon' be making fried arepas?

- My beautiful arepa!

- I'm so glad we did this

I think we should let this cook for about 3 and a half minutes on this side

- Alright

- Pull them off, let them cool

Here we have our half and half

That I put the arepas that we made in there

And here we have our milk

And I put the arepas that we made in here as well

And that's gonna help infuse our ice cream flavor with arepa flavor

So we have our eggs, which is a typical ice cream ingredient

Then we have our sugar

Then we have corn butter

Which is just a simple amalgamation of sugar,

and fat, and corn

I did blend a little bit of it

and then kept the rest of it whole kernel

So you have that texture in the ice cream

- Nice

- And the we have our cheese roux

that is also made from...

Corn flour!

We are just getting it all in!

Of course we have a little bit of salt

Because every ice cream needs salt

Now we combine these

So you are going to hold this

- Oh my God it smells already

like the roasted corn, like that arepa, I can smell it

- Now we are going to strain them

And we will get as much liquid out of this

Because this is going to be our ice cream base

Sounds like a camel peeing

- I pee like that

Are we leaving some of the arepa here for texture?

- We can if you want

Like that much?

Now our pan is pretty hot

If you want to go ahead and...

Then we want to add in the sugar

- Isn't it weird that we're using this as a stove?

- It is a little strange but I think it works

- Sorry we don't have a budget

- And then we are going to add our cheese flavor

We'll add about half of it, and then we'll taste it

- Which cheese are we using here?

- That was Cheddar cheese

Speaking of cheese

The last show I worked on

I made a 17-cheese ice cream

Which was, I thought impossible

But it was quite delicious

And we're going to add a little bit of the corn butter

It's pretty strong in corn flavor

So I don't want to add too much

A quick tip in cooking is that

you can always add more

But if you add too much you've got to start over

You can keep stirring

What we're going to do is add our egg yolks

And this will help our ice cream set up

Essentially what we're doing is we're making a hot custard

And then we're going to cool it and spin it

Because all ice cream is is a custard

- It's so good

It's so corn forward

I can feel the arepa

- I think maybe we should add a little bit more cheese

- Alright

- Ok? Just a little bit more

- Is this going to make it salty?

- It's going to make it a little more cheesy

because there is a good cheese flavor in there

It's kind of muted

I forgot to add a little bit of the salt

And that will help bring up the flavor in the corn

Mmmmm

- Yeah?

- Better

Better

We're double dipping but it's OK because we know each other

From Craigslist

Don't double dip with strangers

A little bit better, no?

- Ooohh

Guys, I came from Puerto Ordaz

And I'm [making] arepas in Los Angeles

What the ****!

How long do we need to be doing this for?

- Probably about 30 more minutes

We'll stir this and then it will thicken

And then we'll let it cool

And then we'll spin it into ice cream

- OK something I haven't told you

- OK, ready

- Is that somebody's actually going to come and taste this flavor

- What?

- Yeah, we actually have guests coming to...

...here, to taste whatever that we make

- Dude! I have no idea...

The last thing I need is people judging me

on something I'm pulling literally out of my a**

I'm sorry for the visual

But luckily, they're going to be Americans

So...

- No the people that are coming are actually Venezuelans

- Oh my God!

Why didn't you tell me?

- OK we put that batter in the freezer

And we're going to let it chill for how long?

- About 30 minutes

Just to speed up the chilling process

2 and a half cups of milk

Half a cup of sugar

Half a tea spoon of salt

3 avocados

and the juice of half a lime

First, you're going to take the avocados

Cut them in half

What you want to do is add about half of the milk to the avocado

and then we're going to spin it in the Cusinart

There we go, good job!

- I'm not a dog, Tess

- Okay

- Alright

Perfect!

- Oh my God!

- Nice and smooth

Now we're going to add this to the pan

Turn this on

And we're going to add all of our ingredients

And that's it

- That's it? That's the ice cream?

- That's the ice cream

We're going to stir it up

Then we're going to let it chill

Then we're going to spin it

Into la crema

La ice cream

We're going to pour the ice cream base

Into the ice cream maker

- The moment of truth

- Just let it spin for about an hour

- OK so we put that ice cream in the freezer

And we're going to leave it there for...

- Overnight

- And then? What are we going to do?

- Then we're going to make our arepa ice cream sandwiches

- Guys, it's the second day

And we're going to try to make a plantain sauce

Because that's the way that you eat an arepa

With cheese, avocado and plantains

- I totally agree

You are the Venezuelan

So, I defer to to you

- OK what do we need for that?

- Now we have to chop our plantains

I'm going to actually slice my plantain pretty thin

Because it's a topper

As opposed to a featured ingredient

Artisanal ice cream sandwich has 'accoutrement'

- What the f*** is that?!

- That's French

- You know, a little bit extra

- The extra mile

- Ahhh, you mean 'accoutrement'?

- Ohhh, she speaks French!

- This is going to be a super heavy ice cream

Because we have cheese, we have avocado

And now we're going to [make] a plantain topping

- This is true

- We didn't tie our hair the whole time

So I really hope we don't find any hairs

- We're just going to caramelize this sugar

- We're using the griddle for everything

because i need to lose about 20 pounds

So I haven't had arepas for about...

- These are ingredients that you would never expect to be cold or sweet

But this is an ace cream!

El boxeo sigue insistiendo en este modelo de negocios para sus principales peleas, y con un precio de 84

95 dólares por vivienda para mirar la función, muchos esperarán que las expectativas se coronen para justificar el costo

 No se sabe si ello ocurrirá sino hasta que ambos contendientes lleguen al cuadrilátero para sostener un pleito previsto originalmente para mayo, antes de que el mexicano Álvarez diera positivo de dopaje

important aspect of who you are we all

want to feel significant but it doesn't

necessarily have to be connected with

how much or how great we achieve certain

things think about it the way you are

thinking it's not unjustified it's

totally ok it's normal to want to be

significant because look at the state of

things in this day and age it's not easy

to be considered popular or to be

someone of value if we all want to be

that influencer we want to be that

celebrity we want to be acknowledged for

doing something great with our lives and

being this person having this persona

that is what we see day in and day out

on social media on television we all

want to

be that person and yet we forget who we

are on the inside who we actually are as

people and yet these achievements all

these acts of significance they are the

only thing that make you feel valued

again it's not that it's wrong to think

this way what's wrong is that there is a

chink in your armor you are incomplete

with your thought process in reality

what the situation is is that you are

actually putting yourself into a corner

where without a there is no be without

being a certain way I am NOT of value if

I do not win the tournament I am no good

as a person there is no point in doing

what I have been doing without earning a

certain amount of money I am no good my

life is kind of meaningless I'm not able

to provide for my family because I do

not earn a certain amount and most of

the time this is usually a very

arbitrary form of thinking we aren't

really able to give a finite answer we

can't put a number on it yet our heads

are in the clouds with regard to this we

constantly put ourselves into this frame

of mind that without this being a

certain way things cannot exist for me

my identity is strongly linked with how

I perform and if I fail to perform a

certain way I am not meeting the needs

of my identity again I am trying to tell

you that look at the link between these

two parts this is your level of

performance at this point of time now

let's say it's bad and your identity is

someone that hey I'm a high performer I

don't deserve this I don't really want

to be in this place what can you do

to either change your performance to

make it good or look at it like this

this is who you are your identity being

a high performer now it's not meeting

the needs of your identity of being a

high performer that's why there's a

conflict that's why you do not feel

worthy enough and that's the case with

most of us where we attach our value

only to our performance and in India

especially this is the case look at most

students they aren't happy because they

aren't ranking in their class they

aren't happy if they aren't performing

in extracurricular activities in sports

in competitions in musical performances

or my friends are doing so much they're

doing so well why can't I achieve the

same thing why can't I be like them and

the rat race pushes us into this frame

of mind where I need to be like everyone

else

and we forego our own good qualities our

own self worth coming back to this you

have two options for yourself either

improve your performance and try to make

it inline make it congruent with your

identity or change your identity with

your capabilities or your current level

of performance and then work your way up

from there that's important to consider

because it's not easy to become a high

performer it takes time it takes

practice it takes consistency it takes

focus but developing that strong frame

of mind where hey I want to be this now

how can I go about doing this I have all

these amazing qualities I'm an amazing

person already now how can I go about

changing that and taking it one step

further to become this high performer

and then how can I use the mindset of a

high performer to boost my performance

what are the skills that I need to learn

and inculcate into my day

so that I can actually boost my

performance that kind of thinking makes

a big difference towards actually

performing better and also valuing

yourself a lot more think about it your

parents want you to be happy and they

would have probably raised you to be a

happy person but your competition tends

to dominate you sometimes and you fail

to realize that a I have my own

qualities I am me I have a certain

identity of my own my own unique

identity now how can I use that identity

to actually become a high performer or

perform really well and amplify my

self-worth using that it's a small

change but it's a very different way of

thinking by actually looking at it like

this by telling ourselves hey how can I

actually amplify my performance I go

back to what I have of value what do I

attach to my identity my value is that I

am good at what I am talented at I have

these amazing qualities already in place

now let's say you don't have any good

qualities start thinking about where

your strong points are you don't have

strong points go one step further

beneath and try and understand what

drives you what motivates you what makes

you wake up in the morning and actually

do something with your time important to

consider that's how you slowly scale it

up to actually reach a level of

performance now you reach that level of

performance and yet you don't feel

significant that's again important to

consider because no matter how much

certain some people may achieve in their

life they never experience success and

it's primarily because I see myself of

value only if I achieve and success is a

psychological sense of accomplishment

so you see sportsmen who have gone

through their entire career fail to

reach that pinnacle of their sport and

yet they aren't able to retire

Pease primarily because they feel that

you know what my performance was

unfinished I could have done a little

better

and once they retired now they cannot

look back on that we all have regrets

look at students students who were

deprived of that opportunity to actually

go about you know pursuing their higher

studies getting their dream job you

failed a number of businesses and now

you're stuck you're not able to come out

of that frame of mind so important to

consider because when we look at the

bigger picture it is this if you want to

amplify your performance you want to

actually do what the high performance to

learn from them study like them and then

live like them change the mindset and

then change the behavior that's all you

need to do it's that simple and once you

start doing that you start following

these two principles one being growing

each and every single day and number two

giving it making it more than just being

about yourself your self-worth

immediately increases and see what it

does to your performance you start

elevating each and every single day

because you are making it more than just

about you you're looking at how can I

grow from any bad experience that I have

how can I learn from good experiences

and take that one step further towards

actually performing even better this

frame of mind immediately changes

everything that you do think about it

think long and hard about how you can

actually go about implementing this in

your life who are the experts in your

field do they attach their self-worth to

the performance or do they amplify their

own qualities and strengths to actually

reach where they are today

think about that start growing start

giving and start changing your mindset

with regard to how you are going to

become a different person you are

value is defined by you not by how much

you perform or how well you may perform

in future it just amplifies who you

already are

think about it someone who is wealthy if

they're a good person their good nature

of growing and giving with that money

it increases someone who is bad on the

inside selfish that selfishness is going

to increase

we don't give as much so that is the

most important thing your value on the

inside that is your self-worth ponder on

welcome to another MedCram lecture we

were talking about asthma and

specifically the causes the

pathophysiology diagnosis and treatment

of this very common disease I want to

kind of show you what's going on at the

bronchial level you remember if you look

at our pulmonary function test

discussion we looked at a flow volume

loop and what happens when you blow out

air very quickly your forced expiratory

volume or flow rate goes up very quickly

and then comes down and then you take a

inhalation in and thus obstruction that

occurs in asthma and in COPD occurs in

this area here which is the small

Airways and as that obstruction gets

worse you see a decrement in the flow

rates especially later on in blowing out

and the point here I want to make is

that these are the small Airways so this

is where asthma is occurring it's

obstruction of these small Airways not

the large ones but the small ones and as

such I want a diagram what it is that's

going on so you've got the airway and

this is a very simplified version of

what it is that's going on you've got

this lumen of the airway that's here and

as you may know it is lined with smooth

muscle when that smooth muscle contracts

this lumen gets smaller okay so there's

two major things that are occurring in

asthma or obstruction of these small

Airways is you've got some sort of an

inflammatory process for whatever reason

that's going on here

this inflammation stimulates certain

cascades that occur and it causes smooth

muscle constriction and as a result the

inflammation causes the lumen to get

smaller and smaller and smaller until

finally there's wheezing there's

obstruction and the primary problem is

that you can't get air out of these

small Airways and that is why in an

asthma attack you will see that this

flow volume loop comes down and the flow

rates in these small Airways is almost

non-existent that's when you run into

asthma attacks

now the severity of this contraction is

going to give you the severity of

asthma attack and how we affect this and

what the pathophysiology is of this is

going to determine how this is treated

so let's talk about that a little bit

once again I want to emphasize that in

asthma what we have is reversible airway

inflammation that is opposed to air

reversible which we would see in COPD

where this airway inflammation has

occurred and this constriction has

occurred in COPD it's kind of

irreversible in asthma we can actually

reverse it back to almost completely

normal this inflammation and

constriction can sometimes be mediated

by IgE so sometimes you will see

elevated IgE levels and that fits into

the pathophysiology and the treatment

what is IgE it's an immunoglobulin

specifically it's an immunoglobulin E

and that's important because

immunoglobulin E affects a cascade that

releases histamine which causes swelling

which is going to make the area in that

Bronco even smaller IgE uses the

basophils and mast cells these release

histamine there are ways of preventing

these cells from releasing histamine and

that is sodium chromo gleich 8 you'll

see for instance medications like Chrome

illan sodium criminal glycated is used

to stabilize these mast cells and

basophils and prevent histamine so

sometimes we actually use that in the

treatment of asthma ok so again I want

to review the bottom line here is is

that we've got a situation where you

have inflammation which causes

constriction of the smooth muscles in

the bronchioles so let's talk a little

bit about this smooth muscle then okay

we've got nerves that affect this smooth

muscle two types of nerves specifically

and they have receptors the first one I

want to talk about is the beta receptor

so you have a nerve that come

down and releases compound which hits

this beta receptor and this causes

relaxation and there's another receptor

which is the muscarinic receptor and

that causes constriction and so in both

asthma and COPD as it turns out we want

this muscle to be relaxed that's the

goal we want relaxation and so what

we're going to actually end up doing is

activating this receptor and blocking

this receptor and so what you see is

pharmacologically we're going to be

using anti muscarinic s' and we're going

to be using

beta agonists that's how we get to the

treatment now the other thing that we're

going to see pharmacologically and we'll

talk about this is inflammation

inflammation is going to cause

constriction because of inflammation

so what we're also going to use is

anti-inflammatories so what I've just

shown you here are the three

cornerstones of asthma treatment some of

them are going to be used first because

they're much more effective than others

and you could probably guess which one

is going to be most effective because

inflammation is starting everything off

the anti-inflammatories are gonna be

number one the beta agonists are gonna

be number two and the anti muscular

necks are gonna be number three okay so

we'll get into the pharmacology of that

let's talk about the pathophysiology

about perhaps clinically where this

inflammation may be coming from

things that can trigger problems in our

Airways based on a number of things

first of all because of the stuff that

we breathe in so that's a trigger the

air that we breathe

what else could trigger asthma what else

could be causing it well there's a tube

that sits right next to the trachea

which goes down to the stomach called

the esophagus and sometimes stuff in the

stomach can reflux up and irritate the

airway that continues up so another

thing that can cause this is

gastroesophageal reflux disease that can

sometimes cause symptoms of asthma now

if we go up here we have the nasal

passageway sometimes you can have

allergies that drip down and cause

irritation in there so then post nasal

drip can cause asthmatic symptoms and so

whenever you see a patient with asthma

or you're asked about on a test you

should think about these things as

causing asthma so what we call coffee

variants asthma post nasal drip and

gastro esophageal reflux disease some of

the other things that can contribute to

all of these think about down feather

pillows pets in the bedroom

these are places that you spend a good

six seven eight hours a day and could

affect some of the treatment symptoms so

these are some of the things that by

getting rid of we might improve asthma

there's also environmental things and

things from work for instance from your

occupation there's something called

isocyanates or cotton dust wood dust

solvents all of these things can by

breathing the men set off asthma

exacerbations there's one other thing

that I should make you aware that they

might ask you on a test they love to ask

things that are uncommon in asthma that

could be very unique or make you think

of something and there's something

called samplers triad

where you see number one aspirin

sensitivity number two asthma and number

three nasal polyps now when these people

take aspirin or any type of NSAID what

can happen is that these medications

inhibit cyclooxygenase and as a result

they're going to have increasing

compounds that are going to set off

asthma exacerbations the key here is

that within hours of taking aspirin

they're going to start to feel flushed

and they're going to go into an asthma

exacerbation these patients who are

typically sensitive to aspirin or NSAID

products are also going to have nasal

polyps the key here is that they need to

stop using aspirin and NSAID products

and find an alternative these patients

by the way and we'll talk about

treatment also respond very well to

leukotrienes antagonists and we'll talk

about those

so just remember Sanders try it in your

patients with asthma

ask him if they've had a history of

aspirin sensitivity or if you've noticed

that their symptoms have gotten worse

after taking aspirin or NSAIDs and then

ask them about a history of sinus

problems maybe even sinus surgery or

nasal polyps
